Privacy Policy

We acknowledge your concern about the privacy of your personal information. Any personal information supplied to Easy Therapy is treated in accordance with our Privacy Policy, bound by the Commonwealth Privacy Act 1988. 

Personal Information we collect

We will only collect personal information that is necessary and relevant to our relationship with you, to enable us to provide you with the best possible service and satisfactory experience. Depending on the nature of our relationship with you and the specific service or services you require, we may request that you provide some or all the following information:

  • information that we may require to initially identify you, including your full name, home address and date of birth;
  • information that we can use to contact you, including your phone number and email address; and
  • information that can assist us to contact third parties involved in your care, including contact details of your plan manager, doctor, and other relevant service providers.

We take reasonable steps to ensure that personal information about you is accurate, complete, up-to-date, and relevant for collecting, holding, using, or disclosing. If you believe personal information that we hold about you is inaccurate, out-of-date, incomplete, or misleading, we will, on receipt of your request, take reasonable steps to correct the information.

At the time of collecting personal information, we will remind you of:

  • our contact details;
  • we are collecting your information and reasons for collecting information;
  • if the collection is required or authorised by law;
  • what happens when we cannot collect your information;
  • any third parties to whom we may disclose the personal information;
  • how you can access and correct your personal information;
  • how you can contact us about privacy concerns or make a privacy complaint;
  • how we handle complaints; and
  • whether your personal information is likely to be disclosed to third party overseas recipients, and the countries in which they are located.

How do we collect personal information

Depending on the nature of the service that we are providing you and method of interaction, we collect personal information via phone (e.g. when arranging the initial consultation appointment) via our website (e.g. enquiries and feedback forms), social media pages (e.g. Facebook or Instagram), through online and face-to-face questionnaires, assessments, and interviews for the service being provided, supplementary documents from other service providers that you have elected to supply us (e.g. letters, reports), via electronic communication (i.e. SMS, email, voice mail, online consultation platforms).

Your personal information will not be collected if you are only browsing our website and social media pages, but we do use cookies to better tailor our information and our services to meet your needs. Please see our Terms of Use for more information on cookies. 

Reasons for collecting personal information

Specific services we provide you will determine the need for collecting, holding, using, or disclosing your personal information. We will inform you at the time it occurs. Our main purposes for collecting personal information are:

  • contacting third parties for the coordination, management, and delivery of Easy Therapy services;
  • providing clinical hand overs and feedback to other service providers; and
  • responding to authorities in relation to legal matters.

We may also use your personal information for other purposes, such as:

  • assisting us to run and operate our business, for instance, planning, accounting, record keeping and archiving of information;
  • improving the quality of our services and standard of performance, inclusive of staff training, systems development, developing new services and undertaking research and evaluations; and
  • complying with our legal obligations and regulatory reporting.

You are not obligated to provide us with any of your personal information but if you choose not to provide us with your personal information, we may not be able to provide the specific services that you require.

Access to personal information

Subject to the recognised exceptions to access for organisations contained in the Australian Privacy Principles of the Privacy Act 1988, you have a right to access your information if you wish (subject to any privilege or legal restrictions); and, if it is reasonable and practicable to do so, we will give you access to the information in the manner requested by you. We will respond to requests within a reasonable period after the request has been made.  By law, we may charge you a reasonable administration fee to cover the cost of retrieving and processing the requested information.

There are some limited circumstances in which we may not be able to provide you with access to your personal information when requested. Such circumstances might include where access would pose a serious threat to the life, health, or safety of another person or where such access would unreasonably impact on the privacy of others.

If we deny you access to your personal information for any reason, or if we are unable to provide you with access to your information in the manner requested by you, then we will provide you with a written notice confirming the reason for such refusal and the complaint process about the refusal.

If in the event of disclosing personal information to a person or entity outside of Australia (e.g., a third-party service provider managed outside of Australia) due to increased globalisation of electronic information systems for service providers, we will take reasonable steps in the specific circumstances to ensure that the overseas recipient does not breach Australian Privacy Principles of the Commonwealth Privacy Act 1988. 

Storing of personal information

We endeavour to take reasonable measures to prevent unauthorised access to, modification of, disclosure, misuse, or loss of that
information as required by law, and in compliance with the Commonwealth Privacy Act 1988. We implement data protection procedures (including locks, password-encrypted software, firewalls, and 2-step verification processes) when storing personal information electronically. Any hard copy records are securely filed, and only accessible by authorised Easy Therapy staff.

Should we no longer require personal information about you for any purposes outlined above, or have obtained unsolicited information we have not requested from you, then we will take reasonable steps to destroy the information, or ensure that such information is de-identifiable, as soon as it is practicable.

Subject to an exception for NSW practitioners in relation to the Health Records and Information Privacy Act 2002 (NSW), we are obligated to retain health information about:

  • adults for 7 years from the last encounter we provided them with a service or product; and
  • children, until 25 years of age.

Changes to this policy

It may be necessary for us to review and revise our Privacy policy periodically. We reserves the right to change our privacy policy at any given time.

Complaints about Privacy

If you believe your personal information has been prejudiced or compromised by us, or have concerns about personal information you have provided us, please contact our Privacy Officer by sending your private-related concerns in writing via email to contact.easytherapy@gmail.com. We will respond to your matters within a reasonable period of 14 days. Where it is practicable, we will always provide you with the option not to identify yourself when dealing with us. Alternatively, you may elect to use a pseudonym to protect your identity, when raising concerns.
